The Challenges Facing Healthcare Today

Healthcare is moving away from the traditional fee for service payment model. Increasingly healthcare providers and networks are being asked to assume risk for not only individual patient outcomes but also for populations of patients often without a full understanding of the incidence of potentially difficult-to-treat and expensive healthcare problems. Certain behavior profiles are a multiplier for healthcare costs.
Healthcare is often reactive and traditionally has not focused on proactive interventions that can prevent increasingly higher levels of care acuity. Other risks include performance mandates, prevention of no shows for appointments, reducing administrative costs, standardized metrics for customer service, communication, accessibility to services, coordination of care after hospitalizations, rapid development plans and the prevention of patient leakage from a healthcare system. Healthcare organizations have responded with new and innovative approaches in managing chronic diseases that include interdisciplinary care with members of the healthcare team that is not usually not found in practices, such as statisticians, dieticians, psychologists and exercise physiologists. Other initiatives include outreach programs that find at risk populations through data mining, proactively implementing preventative health strategies, following these patients closely with in between-visit tracking, compliance monitoring and contextual based reminders that are part of behavioral modification programs customized to patient types.
Chronic Pain is one of the most expensive chronic conditions seen today, because of its high incidence, most of chronic pain is not treated by pain specialists. Chronic pain is also a multiplier of healthcare costs, because its effects on mobility, association with obesity and its impact on disability. Effective treatment of chronic pain requires coordinated treatment plans that include specialized expertise in pharmacologic approaches, procedures and implants, psychological resources and rehabilitation. These specialized resources are not widely available and at times require significant amounts of time that a busy physician would find difficult to find. The widespread efforts to simplify treatment of chronic pain has been cited as one cause of the opioid epidemic in the US.
